Job Summary

We are seeking a highly skilled Registered Nurse to join our team as a Cardiac Rehabilitation Nurse. This is a challenging and rewarding role that requires a strong clinical background and excellent communication skills.

Key Responsibilities
  • Manage the day-to-day operations of the cardiac rehab department, ensuring a high level of patient care and satisfaction.
  • Develop and implement individualized exercise programs for patients with cardiac conditions, with the goal of reducing cardiac events and improving quality of life.
  • Collaborate with healthcare teams to educate patients on their condition and coordinate complex care needs.
  • Act as a resource and advocate for patients and their families, ensuring a maximum level of independence.
  • Work closely with multidisciplinary teams to identify patients with heart failure-related exacerbations and coordinate visits with specialists and primary care physicians as needed.
  • Assess patient understanding of their disease process and ability to carry out instructions provided by the RN or clinic staff.
  • Establish trusting relationships with patients, caregivers, clinic staff, and providers through regular communication and follow-up regarding patient needs and concerns.
  • Utilize motivational interviewing techniques to provide effective education and encourage adherence to recommended healthcare regimens.
  • Make referrals to outside sources as necessary.
  • Research specific clinical situations and collaborate with multidisciplinary care teams to ensure a well-coordinated treatment plan is formed.
  • Engage patients, families, and caregivers regularly to ensure a well-coordinated treatment plan is implemented and any new needs or concerns are identified in a timely manner.
  • Manage patient caseload efficiently and effectively, utilizing time management skills to facilitate the total work process.
  • Provide constructive information to minimize problems and increase customer satisfaction.
  • Seek ways to improve job efficiency and make appropriate suggestions following the appropriate chain of command.
  • Enter timely and accurate data into electronic medical records and other applications necessary to communicate patient needs and ensure complete documentation of patient visits and phone calls.
Requirements
  • Bachelor's degree in Nursing or Associate's degree in Nursing combined with five or more years of experience.
  • Current, unrestricted RN license to practice in the applicable state of employment.
  • 2+ years of diverse clinical experience caring for acutely ill patients with multiple disease conditions.
  • Experience related to patient education and/or motivational interviewing skills.
  • Proficient in PC software computer skills such as Microsoft Word, Excel, PowerPoint, and other departmental specific programs/applications.
  • Proven excellent verbal and written skills.
  • Proven planning, organizing, conflict resolution, negotiating, and interpersonal skills.
  • Proven ability to interact productively with individuals and with multidisciplinary teams.
  • Proven independent problem identification/resolution and decision-making skills.
  • Proven ability to prioritize, plan, and handle multiple tasks, demands simultaneously.
  • Ability to travel occasionally.
Preferred Qualifications
  • CCU/ICU experience.
  • Cardiology experience.
  • BSN degree with current, unrestricted RN license to practice in the applicable state of employment.
  • Experience working with geriatric patient populations.
  • Bilingual (Spanish) language proficiency.
Physical & Mental Requirements
  • Ability to lift up to 10 lbs.
  • Ability to push or pull heavy objects >25 lbs with assistance.
  • Ability to sit for extended periods of time.
  • Ability to stand for extended periods of time.
  • Ability to use fine motor skills to operate office equipment and/or machinery.
