Job SummaryWe are seeking an experienced Pediatric Home Health Nurse Specialist to join our team. The successful candidate will be responsible for providing direct patient care and supervision of LPNs and HHAs in accordance with established plans of care. This role requires strong clinical skills, excellent communication abilities, and a commitment to delivering high-quality patient outcomes.
Key Responsibilities:- Provide comprehensive nursing care to pediatric patients in their homes, focusing on prevention, early detection, and management of acute and chronic conditions.
- Develop and implement individualized plans of care that address patient needs, goals, and physician-ordered services.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and optimal patient outcomes.
- Supervise LPNs and HHAs, providing guidance and support as needed to ensure delivery of high-quality care.
- Bachelor's degree in Nursing (BSN) or higher; Master's degree preferred.
- Current RN license in the state where you work and reside; or eligibility for licensure by examination or endorsement in the NLC states.
- At least 2 years of experience in pediatric home health or similar setting, with a focus on case management and collaboration with interdisciplinary teams.
- Excellent clinical skills, with proficiency in assessing patient needs and developing effective plans of care.
- Strong communication and interpersonal skills, with ability to work effectively with patients, families, caregivers, and healthcare professionals.
- Competitive salary: $65,000 - $90,000 per year, based on experience.
