Job Summary

The Registered Nurse Transfer Center is a critical role within CHRISTUS Health, responsible for ensuring seamless patient transfers into our system. As a key member of our team, you will be responsible for evaluating, coordinating, and documenting patient transfers, as well as collaborating with healthcare partners to determine the best course of action for each patient.

Key Responsibilities
  • Evaluate and coordinate patient transfers into the CHRISTUS Health System
  • Collaborate with healthcare partners to determine the best level of care and transport for patients
  • Document patient transfers and communicate with referring and receiving physicians
  • Utilize problem-solving and customer service skills to determine the best course of action for patients, physicians, and hospitals
  • Work closely with facility House Supervisors, referring physicians, ED, and inpatient staff to ensure efficient patient transfers
Requirements
  • Associate's Degree in Nursing
  • Full-time position
  • Excellent customer service skills, including telephone and personal contact with healthcare teams
  • Ability to escalate concerns and barriers that require administrative judgment
  • Proficiency in computer systems and databases for patient care and data collection
