Job Title: Community Health Worker

At Bonton Farms, we are seeking a highly motivated and compassionate Community Health Worker to join our team. As a Community Health Worker, you will play a critical role in improving community health by engaging with individuals and families to provide support, education, and resources that promote health and well-being.

Key Responsibilities:
  • Conduct outreach activities to identify individuals and families who may benefit from health and social services.
  • Build trusting relationships with community members to encourage their participation in health programs and services.
  • Provide culturally appropriate health education and information to individuals and groups.
  • Assist individuals in accessing healthcare services, including scheduling appointments, providing transportation assistance, and navigating the healthcare system.
  • Advocate on behalf of individuals and families to ensure they receive appropriate services and support.
Requirements:
  • High school diploma or equivalent; additional education or training in health or social services is preferred.
  • Certification as a Community Health Worker (CHW) or willingness to obtain certification within a specified timeframe.
  • Prior experience in community health, social services, or a related field is desirable.
  • Strong knowledge of the community and its resources.
  • Excellent interpersonal and communication skills.
  • Ability to work independently and as part of a team.
  • Proficiency in relevant languages spoken by the community is an asset.
  • Valid driver's license and reliable transportation is required.
  • Competent computer skills (proficiency in MS Office products is a must)

  • Communication Skills: Strong verbal and written communication skills.
  • Problem-Solving and Advocacy: Strong problem-solving skills and the ability to advocate for the needs of individuals and families.
  • Team Collaboration: Ability to work collaboratively with healthcare providers, social services, and community-based organizations.
