This position serves as a crucial link between patients, their families, and the healthcare team, ensuring seamless continuity of care for individuals at risk of or currently experiencing inpatient services.

The Healthcare Outreach Specialist empowers patients to take charge of their health by providing guidance and support in activities that foster wellness and health improvement. Additionally, this role involves managing larger patient populations and engaging in performance enhancement initiatives aimed at elevating quality and minimizing costs for these groups.

Key Responsibilities:

Care Coordination

  • Maintains an active list of patients requiring care management, overseeing the frequency and method of each patient's sessions, whether through phone consultations or accompanying them to healthcare appointments.
  • Conducts initial assessments to evaluate patients' medical conditions, identifying barriers and challenges related to chronic illnesses.
  • Collaborates with patients, families, and healthcare professionals to create a comprehensive care plan tailored to the patient's needs.
  • Incorporates evidence-based practices into care plans and patient education as necessary.
  • Regularly assesses the effectiveness of the care plan to ensure patients are on track to meet their health goals, making adjustments as new information arises.
  • Communicates any patient concerns to the Clinical Programs Director to determine the best course of action for resolution.
  • Records all care management activities using the designated documentation tools.
  • Participates in bi-weekly meetings focused on complex patient care presentations.

Qualifications:

  • Current EMT/Paramedic Certification.
  • A minimum of three years of clinical experience, particularly in managing patients with chronic diseases.
  • Excellent interpersonal skills with the ability to build collaborative relationships across diverse backgrounds.
  • Strong organizational, analytical, and problem-solving abilities.
  • Effective verbal and written communication skills.
  • Demonstrated work ethic with the capacity to handle multiple priorities, meet deadlines, and work efficiently.
  • Ability to work independently as well as collaboratively within a team with minimal oversight.
  • Proficient in basic Microsoft Office applications.
  • Possession of a valid driver's license.
  • Prior experience in Case Management is preferred.
