Key Responsibilities- Assessment and Planning: Conduct thorough assessments of patients' nutritional needs, develop personalized plans to meet those needs, and collaborate with the healthcare team to ensure effective implementation.
- Nutrition Counseling: Provide expert nutrition counseling to patients, families, and caregivers, taking into account individual educational, cultural, and socioeconomic backgrounds.
- Documentation and Communication: Accurately document patient assessments, plans, and progress in the medical record, and effectively communicate with the healthcare team to ensure seamless care coordination.
- Quality Improvement: Participate in quality improvement initiatives to enhance patient outcomes, compliance with regulations, and customer satisfaction.
- Education and Research: Contribute to the development of patient education materials, recipes, and food products, and participate in research studies to advance the field of nutrition.
- Education: Bachelor's Degree in Nutrition or related field.
- Licensure: Pennsylvania Registered Dietitian License required.
- Certification: Certified Dietitian/Nutritionist (CDN) or eligible for registration through the Commission on Dietetic Registration (CDR).
