Clinical Social Worker

This role focuses on safeguarding and promoting the welfare of patients, providing essential services as needed. As a key member of the healthcare team, you will work closely with patients, families, and community resources to address complex social and emotional needs.
Job OverviewThis position provides a range of clinical social work services to patients and families in various settings, utilizing individual, family, or group modalities. You will work within the guidelines of professional social work ethics and standards, collaborating with a multidisciplinary team to ensure comprehensive care.
Key Responsibilities- Conduct psychosocial assessments to identify patient needs and develop effective treatment plans.
- Provide crisis intervention and support in situations involving suicide, homicide, family violence, elder or child abuse.
- Collaborate with healthcare teams to promote a smooth, coordinated plan of care, addressing psychosocial factors that impact patient recovery.
- Develop and implement in-home care plans, exploring alternatives to in-home care and arranging for placement as needed.
- Provide treatment, therapy, and counseling services, utilizing individual, family, or group modalities.
- Assist patients and families in understanding psychosocial factors that impact their utilization of hospital and community resources.
- Collaborate with healthcare teams to ensure continuity of care, formulating and implementing psychosocial components of outpatient care.
- Consult with and provide psychosocial support to patients' families and caregivers, as needed.
- Participate in social work professional education, supervision, and teaching activities, supervising or training staff social workers, student interns, and other healthcare professionals.
- Conduct quality improvement activities and research regarding service delivery and effectiveness of interventions.
- Master's degree in Social Work.
- Licensed Clinical Social Worker (LCSW).
- One (1) year of related experience.
- Three (3) years of related experience.
- Normal office setting.
- Possesses and applies the skills and knowledge necessary to provide care to patients throughout the life span.
- Knowledge of casework process and community resources in relation to health, illness, and disability.
- Knowledge of current social work theory and practice, the ability to apply treatment and case management methods, and familiarity with community issues and resources.
- Understanding of organizational theory and practice in large systems and the knowledge of social and health care policies and systems.
- High degree of diagnostic, treatment, interpersonal, organizational, and communication skills.
- Analytical skills required assessing patient needs, to develop associated treatment modalities and to provide effective counseling.
- Computer skills and knowledge of general office programs, spreadsheet programs, database programs, and presentation programs.
- Ability to read/write and communicate in English.
- Excellent interpersonal skills.
- Ability to function well in very busy situations.
- Responsible and reliable.
- Good organization skills.
