As a Physical Therapist with CenterWell Primary Care, you will work closely with patients, families, and healthcare teams to provide high-quality physical therapy services in the home setting.

Key Responsibilities- Plan and administer prescribed physical therapy treatments and training programs for patients with various injuries, illnesses, and functional disabilities.
- Test/screen patients' physical strengths to assist physicians in evaluating their level of function and develop or pursue treatment programs.
- Develop/implement conditioning/rehabilitation programs consistent with physicians' plans of treatment and overall patient goals.
- Confer with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising therapy treatment programs.
- Provide Physical Therapy Assistants and Home Health Aides with written instructions/care plans reflecting current plans of care as related to therapy and supervise/evaluate staff performance.
- Accurately document patients' care observations, interventions, and evaluations, ensuring timely submission of physician-signed orders and implementation of care plans.
- Report patients' progress to physicians, Clinical Managers, staff, patients, and families, submitting evaluation, treatment plans, progress reports, and discharge summaries to supervisors and care management staff.
- Degree from an accredited Physical Therapy Program (approved by the APTA).
- Current and unrestricted Physical Therapy license.
- Minimum of one year of experience in home health or related field preferred.
- Strong organizational and communication skills.
The estimated salary for this position is $96,450 - $133,300 per year, based on qualifications and experience. Our comprehensive benefits package includes paid time off, 401(k) retirement savings plan, employee assistance program, business travel insurance, and accident insurance.
