MDS Coordinator

Cottonwood Healthcare and Rehabilitation Center is seeking a skilled Registered Nurse to join our team at a large senior living community. The ideal candidate will have a graduate degree from an accredited nursing program and a valid, unrestricted RN license in the state. Previous experience in Long Term Care is preferred, but not required. The successful candidate will be responsible for assessing and evaluating the quality of care being given to long-term care residents, performing initial interviews and assessments, developing care plans, and conducting meetings with multidisciplinary treatment teams and families. Responsibilities:

* Assess and evaluate the quality of care being given to long-term care residents
* Perform the initial interview and assess new residents
* Develop a care plan that has long-term and short-term goals for improvement
* Conduct meetings with multidisciplinary treatment teams, caretakers, and families
* Reach out to specialists within the community to get help in achieving care plan goals

Qualifications:

* Graduate of accredited nursing program
* Must have a valid, unrestricted RN license in the state
* Previous MDS experience preferred
* Previous experience in Long Term preferred, but not required
