About the Role

Colorado Visiting Nurse Association is seeking a highly skilled Home Health Admissions Coordinator to join our team. As a key member of our admissions team, you will play a critical role in ensuring the smooth intake of referrals, admission of patients, and scheduling of clinicians.

Key Responsibilities
  • Provide exceptional customer service to internal and external customers regarding Home Health services.
  • Coordinate the intake of referrals, admission of patients, and scheduling of clinicians according to admission criteria and appropriateness.
  • Collaborate with team members to ensure timely patient care and regulatory compliance.
  • Manage multiple referral portals simultaneously and respond to referral partners in a timely manner.
  • Verify staffing availability, service area, and insurance within network, and collaborate with manager for clinical appropriateness.
  • Identify trends in non-compliance and perform several scheduling functions, including daily/weekly scheduling for assigned teams.
  • Utilize effective organization, time-management, and prioritization skills to handle multiple tasks from multiple sources in a busy setting.
  • Facilitate the flow of patient information and communication, and accurately assign new referrals to caregivers on a timely basis.
Requirements
  • High school diploma; degree from an accredited university in medical, business, or human relations preferred.
  • One (1) year clerical/customer service experience/data entry (home health preferred); experience in extrapolating referrals from portals preferred.
