Job Summary

We are seeking a skilled Physical Therapist to join our Home Health team. As a Physical Therapist, you will play a vital role in helping patients recover from illness or injury and regain their independence.

Responsibilities
  • Conduct comprehensive evaluations to assess patients' physical abilities and develop personalized treatment plans.
  • Provide one-on-one therapy sessions to patients in their homes, using evidence-based techniques and modalities.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Supervise and mentor physical therapy assistants as needed.
Requirements
  • Bachelor's degree in Physical Therapy or equivalent required; Master's degree preferred.
  • Current Physical Therapy licensure in the state of practice required.
  • Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification required.
