Job Title: Care Transition Navigator

We are seeking a skilled Care Transition Navigator to join our team. As a Care Transition Navigator, you will be the face of VitalCaring Group when interacting with referral sources, ensuring a smooth transition of patients to our care.

Key Responsibilities:
  • Contribute to a culture of accountability and teamwork
  • Deliver exemplary care to patients along the care continuum
  • Cultivate and nurture relationships with referral sources
  • Champion communication among and between care teams
  • Own individual sales goals
Requirements:
  • RN license, valid state driver's license, and reliable transportation
  • Two years clinical experience; one year of home health or healthcare sales experience preferred
  • Knowledge of business and fiscal management, governmental regulations, and accreditation standards
  • Established healthcare contacts with data of achieving referral goals within the market preferred
