Job Title: Care Transition NavigatorWe are seeking a skilled Care Transition Navigator to join our team. As a Care Transition Navigator, you will be the face of VitalCaring Group when interacting with referral sources, ensuring a smooth transition of patients to our care.
Key Responsibilities:- Contribute to a culture of accountability and teamwork
- Deliver exemplary care to patients along the care continuum
- Cultivate and nurture relationships with referral sources
- Champion communication among and between care teams
- Own individual sales goals
- RN license, valid state driver's license, and reliable transportation
- Two years clinical experience; one year of home health or healthcare sales experience preferred
- Knowledge of business and fiscal management, governmental regulations, and accreditation standards
- Established healthcare contacts with data of achieving referral goals within the market preferred
