Responsibilities
  • Provide direct patient care in a Hospice setting, including administering medications, wound care, and other treatments as needed.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Conduct thorough assessments of patients' physical, emotional, and social needs.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Document patient progress and outcomes in accordance with regulatory requirements.
Requirements
  • Current RN licensure in the state of employment.
  • Minimum of 1 year of experience in a Hospice or similar setting.
  • Ability to work 40 hours per week, with flexibility to work varying shifts.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
