RN Case Manager

The RN Case Manager, Long Term Care, is a critical role in our organization, responsible for assessing, implementing, and coordinating a plan of care for residents in our long-term care community. - Minimum of one to two years of professional nursing experience in a clinic care setting.
- Currently holds an unencumbered RN license with the State Board of Nursing.
