Pinnacle Home Care Inc. is seeking a skilled Speech Therapist to join our team of healthcare professionals. As a Speech Therapist, you will play a vital role in providing high-quality care to our patients in their homes.
Key Responsibilities- Evaluate patients' level of functioning and hearing, and recommend mechanisms to enhance their hearing ability.
- Conduct comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
- Develop and implement individualized Speech Therapy treatment plans in consultation with physicians.
- Collaborate with other healthcare professionals to develop and implement interdisciplinary client care plans.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care delivery.
- Document all evaluation data, treatments, and patients' responses to therapeutic interventions.
- Supervise Home Health Aides as needed and ensure they are equipped to provide high-quality care.
- Report patients' progress and any changes in their condition to physicians and other healthcare professionals.
- Graduate of an accredited Speech Therapy program or equivalent education.
- Licensed in the State of Florida as a Speech Therapist.
- Two years of clinical experience as a Speech Therapist.
- Current BLS Certification.
- Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
- Ability to work independently with minimal supervision and possess good observational, judgment, and communication skills.
