Job Summary

We are seeking a skilled Nurse Practitioner to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments, medical history reviews, and provide health education to patients in their homes. You will work collaboratively with physicians, case managers, and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.

Responsibilities
  • Conduct adult/geriatric assessments, medical history reviews, and provide health education to patients in their homes
  • Work collaboratively with physicians, case managers, and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes
  • Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up care
  • Provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits
  • Document patient information accurately and thoroughly, following Matrix Medical Network guidelines and protocols
Requirements
  • Master's degree in Nursing or related field required
  • Current RN and NP licensure in state of practice, with prescription authority or ability to obtain
  • Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health record software
  • Excellent verbal and written communication skills
  • Ability to travel to patients' homes and other locations as needed
