KEY RESPONSIBILITIES:
- CLINICAL SUPPORT:
- Assess patient eligibility for various service needs.
- Assist patients in obtaining necessary medical, social, educational, and employment resources.
- Evaluate the effectiveness of recommended services and make adjustments as necessary.
- TREATMENT PLANNING AND MONITORING:
- Create personalized integrated action plans for each patient.
- Connect patients and their families with available resources, monitor compliance with recommendations, and follow up as needed.
- Ensure patients are receiving care from healthcare professionals outside of BrightView as per the treatment plan.
- Provide crisis intervention and de-escalation strategies.
- COORDINATION AND COMMUNICATION:
- Guarantee reliable transportation for patients to BrightView and other healthcare providers.
- Participate in required meetings, including individual and group supervision, treatment team discussions, and quarterly training sessions.
- DOCUMENTATION AND COMPLIANCE:
- Prepare comprehensive reports on needs assessments and care coordination activities.
- Accurately document all necessary care coordination information in the electronic patient health record, adhering to regulatory and company compliance standards.
- Exceptional verbal and written communication abilities.
- Empathetic and compassionate approach to support the recovery process of patients.
- Ability to work effectively with a diverse group of colleagues and patients.
- Proactive problem solver, dedicated to meeting patient needs with urgency.
- Collaborative team player in a multidisciplinary healthcare environment.
- Adaptable and flexible in a dynamic work setting.
- Technologically proficient, comfortable using various systems for communication and documentation. Familiarity with MS Office applications preferred.
- Commitment to compliance and integrity in work standards and regulatory requirements.
- Understanding of HIPAA and 42-CFR Part 2 regulations is preferred.
- Experience with harm reduction strategies is a plus.
- EXPERIENCE:
- Minimum of 1 year of experience in substance use services or clinical experience with individuals having co-occurring SUD and mental health conditions.
- EDUCATION:
- Bachelor's Degree in Social Work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation, or Human Services is required.
