About the Role:
VNS Health is seeking a skilled Registered Nurse to join our Home Health Infusion team. As a Home Health Infusion Nurse, you will provide high-quality patient care and deliver infusion therapy services in the comfort of patients' homes.
Key Responsibilities:
- Assess patient needs and develop individualized care plans to ensure effective and safe infusion therapy.
- Administer infusion therapy, including venous blood draws, central line blood draws, peripheral line placement, and IV medication administration.
- Provide education and training to patients and their families on infusion therapy and medication management.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Document patient care and treatment plans in a timely and accurate manner.
- Participate in ongoing education and training to stay up-to-date on the latest infusion therapy techniques and best practices.
Requirements:
- Current licensure as a Registered Nurse in New York State.
- Minimum of two years of experience in a clinical setting, with at least one year of experience in infusion therapy.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a focus on delivering high-quality patient care.
- Basic Life Support (BLS) certification.
