Job Summary

UnitedHealth Group is seeking a skilled Nurse Practitioner or Physician Assistant to join our Home-Based Medical Care team in Schenectady, NY. As a member of our interdisciplinary team, you will provide high-quality, patient-centered care to complex, chronic patients in the comfort of their own homes.

Key Responsibilities:
  • Conduct comprehensive assessments and develop personalized care plans for patients
  • Collaborate with primary care physicians, facility staff, and other healthcare professionals to ensure seamless care coordination
  • Prescribe appropriate diagnostics and interventions to prevent unnecessary hospitalizations
  • Provide education and support to patients and their families on disease management and wellness
  • Participate in quality improvement initiatives and contribute to the development of best practices in home-based medical care
Requirements:
  • Current state RN license (if Nurse Practitioner) or NP/PA license
  • Completion of accredited Nurse Practitioner or Physician Assistant program
  • Knowledge of long-term care, community resources, and cost-effective alternatives
  • Access to reliable transportation
Preferred Qualifications:
  • 2+ years of clinical experience
  • Knowledge of CPT, ICD-9, and HCPC codes
  • Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems
