About the Role

We are seeking a skilled Hospice Registered Nurse (RN) Case Manager to join our team at Resource 1 Homecare Staffing. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality, patient-centered care to individuals and their families in their homes and communities.

Key Responsibilities
  • Assessment and Planning: Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
  • Care Coordination: Coordinate patient care with other healthcare professionals, including physicians, therapists, and social workers, to ensure seamless transitions and optimal outcomes.
  • Education and Support: Provide education and support to patients and families on disease management, symptom control, and end-of-life care.
  • Communication: Communicate effectively with patients, families, and healthcare teams to ensure that patient needs are met and that care is coordinated.
  • Documentation: Maintain accurate and timely documentation of patient care, including progress notes, treatment plans, and other relevant information.
Requirements
  • Education: Graduation from an accredited school of nursing.
  • Experience: 12 years of recent acute care experience in an institutional setting (required). 12 years of recent home care experience (preferred).
  • Skills: Strong assessment and planning skills, excellent communication and interpersonal skills, ability to work independently and as part of a team, and proficiency in electronic medical records (EMRs).
