Job Summary:
As a Home Health LPN, you will play a critical role in providing direct skilled nursing services to patients in their homes. Your responsibilities will include maintaining coordination of staffing assignments, ensuring compliance with physician orders and applicable regulations, and providing patient education and support.

Responsibilities:
  • Provide direct skilled nursing services to patients in their homes, as described in the patient's plan of care.
  • Maintain coordination of staffing assignments to meet patient needs.
  • Ensure compliance with physician orders and applicable regulations.
  • Provide patient education and support to promote healthy outcomes.

Requirements:
  • LPN Diploma
  • Active NC LPN License or compact state
  • American Heart Association Basic Life Support (BLS) Certification
  • Experience and/or formal training and education in home care is preferred
  • Must maintain a valid North Carolina driver's license or be licensed in the state you reside, and maintain the ECU Health required minimum insurance limits (clinical home team)
  • Must maintain adequate/dependable transportation in order to perform specific job functions
