We are seeking a skilled Physical Therapist to join our team at HexaQuEST Health, Inc. as an Outpatient Rehabilitation Therapist. As a key member of our rehabilitation team, you will be responsible for providing high-quality physical therapy services to patients in various settings, including outpatient clinics and skilled nursing facilities.
Key Responsibilities- Conduct diagnostic assessments and develop treatment plans for patients with a range of conditions and injuries.
- Provide physical therapy services, including exercises, modalities, and education, to patients in outpatient and inpatient settings.
- Collaborate with other healthcare professionals to ensure comprehensive care and optimal patient outcomes.
- Maintain accurate records and documentation of patient progress and treatment plans.
- Participate in departmental meetings and contribute to quality improvement initiatives.
- Current licensure as a Physical Therapist in the State of Hawaii.
- Minimum 6 months of experience in an outpatient or inpatient rehabilitation setting.
- Ability to work variable shifts, including weekends and holidays, as needed.
