Hospice Care Overview

The RN Nurse Clinician provides holistic care to terminally ill patients and their families by assessing their needs and planning, coordinating, and implementing the Plan of Care.

This role works collaboratively with the interdisciplinary hospice group to anticipate changes in the patient's condition, avoid crises, promote patient and family choice, ensure continuity of care, and enhance the quality of life of dying patients.

Key Responsibilities

  • Assess patient needs and develop individualized care plans
  • Collaborate with the interdisciplinary team to coordinate care
  • Implement and revise the Plan of Care as needed
  • Provide education and support to patients and families

Requirements

  • Associate's or Bachelor's Degree in Nursing
  • Washington Registered Nurse License
  • 1 year of nursing experience, with some pediatric experience required
