Job Summary

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our clinical care team in Mapleville, RI. As a Geriatric Care Specialist, you will deliver in-home care through a blend of in-person visits and virtual visits, working collaboratively with our multidisciplinary care team to provide comprehensive primary care, urgent care, and chronic condition management.

Key Responsibilities
  • Deliver clinical care flexibly, whether in-person at the patient's home or virtually from your own home
  • Provide compassionate, individualized, and holistic patient care, including comprehensive primary care, urgent care, and chronic condition management
  • Collaborate with our multidisciplinary care team to cooperatively deliver transitions of care support, social determinants of health interventions, and complex care management
  • Benefit from professional development opportunities through dedicated clinical education programming and active engagement with our on-staff world-class experts in geriatric medicine, palliative care, geriatric psychiatry, clinical pharmacy, and complex care management
  • Participate in after-hours on-call coverage infrequently throughout the year
Requirements
  • Nurse Practitioner or Physician Assistant with graduation from an accredited health professions school and active national board certification
  • Active licensure in the state of employment
  • Active DEA licensure and state-specific controlled substance registration, as applicable
  • Advanced training in geriatrics and/or palliative medicine preferred
  • Certification in basic life support (BLS) required
  • Clinical experience caring for aging adults and adults with complex care needs required
  • Clinical experience caring for patients in the home setting, including private homes and/or congregate living facilities preferred
  • Clinical experience providing virtual care via telehealth
  • Previous experience with managed care and/or value-based healthcare delivery preferred
  • Average to advanced computer and software skills required
  • Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation
