Job Overview:

This full-time RN position involves assessing patient needs, developing care plans, and coordinating transitions of care. You'll work closely with patients, families, and healthcare teams to ensure seamless care delivery and make a meaningful difference in the lives of those we serve.

Essential Responsibilities:

  • Assess patients' physical, emotional, and spiritual needs through comprehensive evaluations and conversations
  • Develop and implement personalized care plans that address patient goals and preferences
  • Collaborate with healthcare teams to coordinate care transitions, manage medications, and address medical concerns
  • Provide education and support to patients and families on hospice care, pain management, and symptom control

Requirements and Qualifications:

  • Current RN licensure in Ohio
  • Minimum 2 years of experience in acute care or hospice settings
  • Excellent communication and interpersonal skills
  • Ability to work independently and collaboratively as part of a high-functioning team
