As a Home Health Occupational Therapist at CenterWell Primary Care, you will play a vital role in helping patients manage chronic conditions or recover from injury, illness, surgery, or hospitalization.

Our care teams include nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers - all working together to help patients rehabilitate, recover, and regain their independence.

Key responsibilities include:

  • Assessing and screening patients' daily living and work-related skills to develop therapeutic retraining programs with measurable objectives.
  • Administering and interpreting diagnostic and prognostic tests to assist physicians in evaluating patients' levels of function.
  • Conferencing with physicians and other healthcare team members to develop and revise plans of care.
  • Providing therapeutic treatment and instruction to patients in accordance with physician orders to improve or restore strength, coordination, range-of-motion, and function.
  • Reviewing and expanding retraining programs to align with patients' behavioral goals and the overall rehabilitation program.

Requirements include:

  • Degree from an accredited Occupational Therapy program.
  • A minimum of six months of occupational therapy experience, with home health experience a plus.
  • Current and unrestricted OT licensure.
  • Current CPR certification.
  • Good organizational and communication skills.
  • Valid driver's license, auto insurance, and reliable transportation.

CenterWell Primary Care offers a competitive compensation package, including a pay range of $84,600 - $116,400 per year, as well as benefits such as paid time off, 401(k) retirement savings plan, employee assistance program, business travel, and accident insurance.
