Responsibilities


Home Health with Reduced Travel


Our Registered Nurses (RNs) supervise, assess, and deliver nursing services to clients in various senior living environments, including assisted living and independent living facilities.


Our facility-based RNs appreciate:
  • Providing care to our patients within the welcoming environment of the assisted living facilities where they reside

They also enjoy the ability to follow each patient from admission to discharge.


Enhabit RNs collaborate daily with facility wellness staff to ensure effective communication and overall continuity of care for residents and their families.

Additionally, our facility-based RNs consult with physicians and the office as needed, providing updates on patient care and serving as the primary contact for all disciplines involved in patient care.

Key responsibilities of our RN Case Managers include accurate OASIS collection, maintaining current medication profiles, timely reporting of lab values to physicians, and attending weekly case conferences and monthly case manager meetings.

Qualifications


Registered Nurses (RN) Case Managers must meet the following criteria:

  • Current licensure as a Registered Nurse (RN) in the state of employment
  • A minimum of one year of clinical experience is preferred
  • Demonstrated knowledge and skills in current nursing practices
  • Possession of a valid state driver's license and automobile liability insurance
