This preventive care initiative helps identify and mitigate health risks, ensuring patients receive coordinated follow-up care to improve their overall well-being. By joining our team, you'll be part of a seamless health journey that connects care across various settings.

Key Responsibilities:
  • Conduct in-home assessments, including medical history evaluation, system review, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing (as appropriate).
  • Identify diagnoses for care management and active medical management in the furtherance of treatment.
  • Formulate a list of current and past medical conditions using clinical knowledge and judgment, along with assessment findings.
  • Communicate assessment results to inform members' primary care physicians of potential care gaps.
  • Recognize urgent and emergent situations and intervene accordingly.
  • Educate members on disease processes, medication, and compliance.
Requirements:
  • Active NP or PA license or ability to obtain by start date, with unencumbered licensure and no open/unresolved disciplinary actions.
  • Active national NP or PA certification or ability to obtain national certification in the state of assignment by start date (ANCC or AANP national certification for NPs or NCCPA certification for PAs).
  • Ability to complete physical job requirements, including lifting a 30-pound bag, navigating stairs, and various dwelling conditions.
  • Access to reliable transportation for up to 85% travel within licensed states (with notice, based on business needs) to complete home visit assessments.
Preferred Qualifications:
  • 1+ years of clinical experience in the highest level of education, clinical setting.
  • Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology.
  • Home healthcare or home visit experience.
  • May be requested to obtain additional licensure in other geographic areas.
Compensation and Benefits:

Washington residents can expect a salary range of $88,000 to $173,200 annually, based on factors such as education, work experience, certifications, and more.
