Job Summary:

- Provide expert nutritional counseling and education to residents, families, and healthcare teams.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and delivery.
Responsibilities:
- Conduct thorough nutritional assessments, including medical history, dietary habits, and lifestyle factors.
- Develop and implement individualized nutrition plans, taking into account resident needs, preferences, and goals.
- Provide ongoing nutrition education and counseling to residents and their families, promoting healthy eating habits and lifestyle choices.

Requirements:
