Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Almost Family - Marinette PD. As a LPN, you will provide high-quality clinical services to patients in their homes, working under the direct supervision of a Registered Nurse and/or designated supervisor.

Responsibilities
  • Provide direct nursing care to patients as prescribed by the physician, including administering medications, wound care, and vital sign monitoring.
  • Document patient visits thoroughly and completely in the electronic medical record, following all applicable infection control procedures.
  • Participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, and other healthcare professionals.
  • Ensure confidentiality of patient health information and maintain a safe and healthy work environment.
Requirements
  • Current LPN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
  • Previous home care experience preferred.
