Job Summary

This is a full-time, permanent opportunity for a Board Certified/Board Eligible Family Medicine, Internal Medicine, or Geriatric Medicine Physician to join our team. You will be responsible for providing comprehensive care to patients with acute and chronic illnesses, working collaboratively with our hospitalist team, and leading a care team consisting of a care promoter, care facilitator, and care coordinator.

Responsibilities
  • Provide high-quality, patient-centered care to seniors with acute and chronic illnesses
  • Work collaboratively with our hospitalist team to ensure seamless care transitions
  • Lead a care team to provide comprehensive care to patients with complex health needs
  • Participate in the management of our center and contribute to a positive culture of love, accountability, and passion
  • Perform other duties as assigned and modified at the manager's discretion
