We are looking for a qualified Clinical Nutrition Specialist to join our team at Denali Dialysis. This role involves providing comprehensive nutritional support to our patients.
Key Responsibilities:
1. Conduct initial Nutritional Assessments and create tailored Nutrition Care plans for each patient.
2. Develop, interpret, and enforce policies related to nutritional processes within the clinic.
3. Monitor ongoing nutritional progress by reviewing monthly lab results and making necessary dietary adjustments, providing recommendations to patients and their primary caregivers to enhance health outcomes.
4. Collaborate as part of a multi-disciplinary team in both long-term and short-term patient care planning.
5. Create educational materials such as food diaries, bulletin boards, and seasonal posters to promote nutrition awareness among patients and staff.
6. Ensure compliance with clinical policies and government regulations regarding nutritional documentation and outcomes, including care plans and quality improvement initiatives.
Qualifications:
- Bachelor of Science in Nutrition or a related field is required.
- Registered Dietitian credential from the Commission on Dietetic Registration (CDR) is mandatory.
- Proficiency in computer software applications.
- Ability to travel to various clinics as necessary.
