Job Title: Health Care Case Manager

Rutgers, The State University of New Jersey, is seeking a skilled Health Care Case Manager to join our team at the School of Nursing in Paterson, NJ. As a key member of our Child Health Program, you will play a vital role in supporting the Department of Children and Families (DCF)/Child Protection and Permanency (CP&P) in meeting the health care and protection needs of children in out-of-home placement.

Key Responsibilities:
  • Assist with enrollment of children in managed care entities and identification of medical homes (primary care providers)
  • Assess children's health care status and needs by retrieving and reviewing medical records and interviewing past providers and caregivers
  • Develop and implement child health care plans in collaboration with medical homes, specialists, caregivers, and community service providers
  • Link children and caregivers with appropriate health resources and community services
  • Ensure timely EPSDT visits and specialty appointments, review reports, and forward necessary information to medical homes
Requirements:
  • Bachelor's Degree in Nursing or a BA/BS degree in a health-related field
  • Minimum 1-2 years of experience in pediatric nursing
  • Current certification in American Heart Association Basic Life Support for Healthcare Providers
  • Valid New Jersey license and vehicle for off-site visits
Preferred Qualifications:
  • Two years of experience in nursing case management, community health nursing, or nursing care of vulnerable families
