Advanced Practice Provider for Diabetic Management

OSF HealthCare is seeking an Advanced Practice Provider to join our team in Diabetic Management. As a key member of our healthcare team, you will be responsible for conducting comprehensive assessments of patients with diabetic disorders, developing and implementing individualized treatment plans, and monitoring and adjusting treatments based on patient progress and lab results. You will also educate patients and their families about endocrine disorders, treatment options, and lifestyle modifications, and collaborate with endocrinologists, primary care physicians, and other healthcare professionals. Additionally, you will perform diagnostic tests and interpret results, prescribe medications and manage medication regimens, provide follow-up care and manage chronic endocrine conditions, and participate in clinical research and quality improvement initiatives. If you are passionate about Diabetic Management and have a Master's degree or higher in nursing or physician assistant studies from an accredited program, current Illinois license as a nurse practitioner or physician assistant, certification by the American Nurses Credentialing Center (ANCC) or the National Commission on Certification of Physician Assistants (NCCPA), and a minimum of two years of experience as an APP, we encourage you to apply.
