- Plan of Care Development: Collaborate with the interdisciplinary team to develop and implement individualized plans of care for each patient.
- Clinical Care Coordination: Coordinate and facilitate the delivery of high-quality, patient-centered care, ensuring seamless transitions and optimal outcomes.
- Communication and Collaboration: Communicate effectively with patients, families, and the interdisciplinary team to ensure that care is coordinated and patient-centered.
- Quality Improvement: Participate in quality improvement initiatives to enhance patient care and outcomes.
- Education: Associate of Science degree in Nursing (ASN) or Master of Science in Nursing (MSN)
- Licensure: Current valid license as a Registered Nurse in the state of Florida
- Experience: Two (2) years of medical/hospital nursing experience, with a preference for pediatric home health care experience
