Key Responsibilities:
- Collaborate effectively with the rehabilitation team and therapy department.
- Conduct thorough evaluations and maintain accurate documentation.
- Provide insightful recommendations that support assessments and communication.
- Deliver therapy in accordance with established treatment plans and prescriptions.
- Implement therapeutic interventions aimed at improving patients' functional abilities.
- Manage treatment workflows, ensuring timely documentation of individual and group sessions in the electronic records system.
- Possess knowledge of the Patient-Driven Payment Model (PDPM).
- Align therapy services with Medicaid and Medicare billing practices.
Qualifications:
- Valid New Jersey Physical Therapist License.
- Experience in long-term care settings, particularly in skilled nursing facilities.
- Strong organizational, interpersonal, and communication skills.
