Job Title: Registered Nurse - Hospice Care Specialist

Job Summary:

Vitas Healthcare is seeking a compassionate and skilled Registered Nurse to join our team as a Hospice Care Specialist. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and their families facing terminal illness and bereavement.

Responsibilities:

  • Coordinate patient care and develop individualized plans of care in collaboration with the Interdisciplinary Group (IDG)
  • Conduct comprehensive assessments of patients and families to identify their physical, medical, psychosocial, emotional, and spiritual needs
  • Provide education and training to patients, families, caregivers, and referral sources on end-of-life care and symptom management
  • Participate in patient care meetings and contribute to the development of the patient's plan of care
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication
  • Document patient information accurately and thoroughly in the electronic medical record
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures
  • Represent Vitas Healthcare in a professional and positive manner, adhering to our values and mission

Requirements:

  • Current licensure as a Registered Nurse in the state where the Vitas program is located
  • A minimum of two years of nursing experience in hospice, home health, or community health in the last five years
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills
  • Strong problem-solving and critical thinking skills, with the ability to prioritize tasks and manage multiple responsibilities
  • Proficiency in electronic medical records and other healthcare software
