Core Responsibilities:
- Perform comprehensive needs assessments and connect families with essential resources
- Provide individual and group rehabilitation services to clients
- Timely submission of progress notes utilizing the agency's electronic health record system
- Offer guidance on managing child behaviors and family stressors
- Assist families in accessing both internal and external support services
- Collaborate with the treatment team to formulate client rehabilitation plans
- Engage in community meetings and represent the agency at various events
- Ensure successful placement and long-term housing stability for families
Required Qualifications:
- Bachelor's degree in social work, psychology, or a related discipline
- A minimum of 4 years of experience in Housing Navigation or rehabilitative services
- Valid California driver's license and auto insurance required
Work Environment:
This position involves a combination of office work and field visits to engage with clients in diverse settings. Daily responsibilities may include travel using a motor vehicle.
Physical Demands:
- Stationary position required for 50% of the time
- Regular operation of office equipment and communication with team members
- Occasional requirement to transport materials weighing up to 50 pounds
- Use of a motor vehicle for business-related purposes
