Job Description

As a Licensed Practical Nurse (LPN) in our Long Term Care team, you will provide high-quality patient care and support to our residents. Your primary responsibility will be to deliver exceptional nursing care, ensuring the comfort, dignity, and well-being of our patients.

Responsibilities:

  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with the interdisciplinary team to ensure comprehensive care and coordination
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments

Requirements:

  • Current LPN licensure in Vermont
  • BLS certification (preferred)
  • Experience in long-term care or geriatric nursing (preferred)
