Job Title: Private Duty Nurse LPN

At Aveanna, we are seeking a dedicated and compassionate Private Duty Nurse LPN to join our team. As a Private Duty Nurse LPN, you will be responsible for delivering high-quality patient care in a home setting, working closely with our patients and their families to ensure the best possible outcomes.

Key Responsibilities:

  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Develop and implement individualized care plans, taking into account the patient's unique needs and goals.
  • Collaborate with the healthcare team to ensure seamless care transitions and effective communication.
  • Document all patient interactions, assessments, and care provided in a timely and accurate manner.

Requirements:

  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Current, unrestricted state license as a Registered Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.

Preferred Qualifications:

  • One year of experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.

Physical Requirements:

  • Ability to speak, write, read, and understand English.
  • Ability to travel.
  • Ability to lift 50 pounds.
  • Prolonged walking, standing, bending, kneeling, reaching, twisting.
  • Ability to sit and climb stairs.
  • Ability to have visual and hearing acuity.
  • Ability to have a strong sense of smell and touch.
  • Ability to sufficiently reposition patients and move equipment without assistance.
  • Ability to appropriately respond physically and mentally to emergency situations in the home or during transport.

Environment:

  • Ability to function in a wide variety of environments, which may involve exposure to allergens and other various conditions.
  • Possible exposure to blood, bodily fluids, and infectious diseases.
