Home Health Physical Therapist Assistant with Patient-Centered Care

Obran Cooperative is seeking a compassionate and skilled Physical Therapy Assistant to join our Home Healthcare Team. As a vital member of our team, you will provide quality home health visits to patients in the comfort of their own homes.
Job OverviewWe are committed to delivering exceptional care to our patients, and we believe that our Physical Therapy Assistants play a crucial role in achieving this goal. In this role, you will work under the direction of a physical therapist and in accordance with a physician's plan of care to provide physical therapy services to our patients.
Key Responsibilities- Provide direct physical therapy to patients according to the directions of a physical therapist and in accordance with a physician's plan of care.
- Treat patients to relieve pain, develop or restore function, and maintain maximum performance.
- Direct and aid patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities, and prosthetic training.
- Utilize physical agents, including heat, cold, water, light, electricity, ultrasound, and massage.
- Observe and report patient reactions to treatment and any changes in patient condition to the physical therapist and Obran Cooperative personnel.
- Instruct patients on care and use of wheelchairs, braces, crutches, canes, prosthetic or orthotic devices.
- Maintain necessary records, including clinical notes and conference notes, which will be incorporated into the patient's clinical record in prescribed time frames.
- Participate in Obran Cooperative's Home Care Team Members meetings and Inservice Education Program.
- Graduate of an accredited physical therapy assistant program with an Associate's degree in science or applied science approved by the American Physical Therapy Association, Inc.
- Current certification or license as required by state law.
- Possesses and maintains current CPR certification.
- Ability to travel to different patients' homes.
- Discretion in dealing with sensitive and confidential information.
Obran Cooperative offers a competitive salary range for this position, with estimated compensation between $40-$50 per visit, plus additional hourly pay for in-services, trainings, and orientations. Mileage reimbursement is also provided for W2 employees.
