About the Role
The Licensed Practical Nurse (LPN) provides high-quality patient care in a skilled nursing facility, working under the direction of a Nurse Case Manager or Clinical Supervisor. This role requires a strong commitment to delivering compassionate, patient-centered care, following established nursing processes and procedures, and adhering to regulatory requirements.
Responsibilities
- Provide direct patient care, including administering medications, performing treatments, and monitoring patient conditions.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Conduct thorough assessments, identify patient needs, and develop effective care strategies.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.
Requirements
- Current Oregon Licensed Practical Nurse (LPN) licensure.
- Basic Life Support (BLS) certification within 30 days of hire.
- 2 years of patient care experience in a skilled nursing facility or medical clinic.
Preferred Qualifications
- Experience in Acute Care, Home Health, or Public Health nursing.
