Position Overview:
The MDS (Minimum Data Set) Coordinator plays a crucial role in generating statistical reports necessary for compliance with relevant legal, accrediting, and licensing standards for healthcare facilities.
The MDS Coordinator gathers and organizes information from health records and personal assessments to evaluate the quality of care and conduct utilization reviews.
The MDS Coordinator collaborates with physicians, nurses, and other healthcare professionals to ensure the accuracy and completeness of medical records.
Key Responsibilities:
- Strategically plans, directs, and coordinates MDS assessments and care plan services.
- Conducts health assessments for all residents upon admission to establish initial care plans and performs additional assessments quarterly.
- Assists in the development, implementation, and evaluation of individualized care plans in collaboration with other healthcare providers and physicians.
- Analyzes and accurately codes diagnoses, operations, and procedures from health records using appropriate classification systems, ensuring precise data entry into the computer system.
- Organizes and leads quarterly care plan conferences for each resident, involving family members and physicians.
- Develops, implements, and coordinates quality management, utilization review, and discharge planning by consulting with other healthcare providers regarding resident care needs.
- Reviews charts for unnecessary documentation quarterly, adhering to established minimum standards for documentation retention.
- Meets all deadlines for reports, records, and documentation as mandated by state and federal regulations.
- Actively engages in the QAPI (Quality Assurance Process Improvement) program, demonstrating a proactive approach to enhancing quality care and services with the involvement of residents, families, and staff.
- Monitors compliance with accrediting, surveying, and regulatory body criteria.
- Participates in continuing education to maintain credentials and enhance expertise in nursing and administration.
- Exhibits a high level of initiative, effort, and commitment to completing assessments efficiently with meticulous attention to detail.
- Stays current in the nursing field through participation in meetings, conferences, and workshops related to resident care and rehabilitation.
- Demonstrates a willingness to perform RN duties as needed.
- Possesses empathy and understanding for the elderly population.
- Creates a positive atmosphere for residents aligned with our core values.
- Follows instructions and completes daily assignments as directed, showing initiative for improvement.

Qualifications:
- Bachelor of Science degree in Nursing preferred.
- Three to five years of nursing experience.
- Active RN/LPN license in good standing.
- Proficiency in clinical software, with specific MDS experience preferred.
- Engages in educational programs to stay updated on best practices in nursing.
Specific Requirements:
- Must possess a current, unencumbered nursing license in the State of Iowa.
- Ability to read, write, speak, and understand the English language is essential.
- Must be capable of making independent decisions when necessary.
- Ability to interact effectively with personnel, residents, family members, visitors, and government agencies.
