Job Summary

We are seeking a compassionate and skilled Registered Nurse to join our team as a Home Care Specialist. In this role, you will provide high-quality, patient-centered care to individuals and families in the comfort of their own homes.

Key Responsibilities
  • Evaluate the physical and emotional needs of patients and develop personalized care plans to address their unique needs.
  • Coordinate medication administration, education, and ensure accurate documentation.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
  • Provide ongoing supervision and assessment of patient and family teaching needs.
  • Make referrals to other services as needed and coordinate with the medical social worker.
  • Supervise home health aides and provide education and support as needed.
  • Provide emotional support and promote independence and responsibility throughout the course of the disease.
  • Actively listen to patient and family concerns and support individual coping mechanisms.
  • Provide services at the time of death and assist with bereavement follow-up as needed.
Requirements
  • Current NC license as a registered nurse.
  • Active patient contact or completion of a nursing refresher course within the past three years.
  • CPR certification.
  • Ability to work a flexible schedule and willingness to work with patients and families regardless of race, color, creed, age, or sexual orientation.
  • Strong communication and interpersonal skills.
Physical Demands
  • Visual acuity to read written documentation and computer tasks.
  • Speech and hearing skills necessary for telephone interaction and face-to-face contact.
  • Ability to sit extended periods of time and perform administrative tasks.
  • Ability to stand, bend, and lift an average of 30 pounds.
  • Manual dexterity with the ability to manipulate office equipment and keyboard for extended periods of time.
  • Ability to walk, squat, kneel, climb, and balance.
Special Requirements
  • Must have an automobile with required insurance coverage and state driver's license.
