We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health RN. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to develop and implement personalized care plans.
Key Responsibilities:- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted care plans.
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
- Collaborate with patients' families and caregivers to educate them on care plans, medication management, and home safety.
- Communicate effectively with healthcare providers, including physicians, therapists, and other healthcare professionals, to ensure seamless care coordination.
- Document patient progress, including notes, assessments, and care plans, in a timely and accurate manner.
- Active Registered Nurse (RN) License in Colorado.
- Graduate of an accredited school or program of nursing.
- At least 1200 hours of experience as an RN in healthcare.
- CPR/BLS Certification.
- Home health care experience preferred.
