Job Description

St. Luke's Health System is seeking a skilled Registered Nurse to join our Home Health team in Boise, Idaho.

As a Home Health Nurse, you will provide comprehensive care to patients in the comfort of their own homes, ensuring they receive the highest quality of care and support.

Key Responsibilities:

  • Provide patient-centered care, assessing physical and psychological needs to improve quality of life
  • Develop and implement individualized care plans, working closely with patients, families, and healthcare teams
  • Conduct home visits, providing education and support to patients and their caregivers
  • Collaborate with interdisciplinary teams to ensure seamless transitions of care

Requirements:

  • ASN or BSN degree
  • 1 year of RN experience, with hospice or acute care experience preferred
  • Current RN license in the State of Practice
  • Basic Life Support (BLS) certification
