Job Description:

At Fenway Health, we're seeking a dedicated and compassionate LPN to join our team of mission-driven nurses in our outpatient community health center. As an LPN, you will play a vital role in providing specialized nursing support for patients and medical providers, phone support and triage, as well as general support for other clinical activities to ensure our patients receive the highest quality care.

Responsibilities:
  • Provide patient triage, ensuring the safe and effective disposition of health-related problems
  • Administer comprehensive evaluations of patient telephone callers, 'Code Blue' events, and walk-in patients, developing care plans and consulting with other disciplines as needed
  • Advocate for patient care as a nurse member of the multidisciplinary medical team
  • Communicate care plans to patients in a timely manner
  • Document triage encounters in medical records, including all appropriate activities and plans

Case Management:
  • Coordinate services to improve patient care, using careful assessment, planning, coordination, and delivery of care, with outcome evaluation of the plan
  • Coordinate follow-up care for acutely ill patients, including primary care and specialty visits, VNA, infusion companies, and prior authorization for services
  • Coordinate preventative services for defined populations, including education, vaccinations, monitoring, and arranging for lab tests and referrals
  • Evaluate adherence to care plans and intervene to promote patients' well-being
  • Document services in medical records, including all appropriate activities and plans

Providing Direct Patient Care:
  • Assist with patient care under the direction of medical providers and/or under nursing scope of practice
  • Provide nursing support to RNs, LPNs, and MAs
  • Place and maintain IV lines
  • Administer medications, vaccinations, and other therapeutic interventions as per medical provider orders
  • Check supplies for nursing patient encounters and notify managers or supply ordering designees of additional stock needed
  • Administer medical tests and procedures, such as EKGs, spirometry, and CLIA, as per protocol and medical provider requests
  • Work with MAs and pharmacy to call in prescription refills or obtain prior authorizations
  • Assist with preventative medicine efforts, such as flu, STD, and vital sign checks
  • Coordinate follow-up care for acutely ill patients, including primary care and specialty visits, VNA, infusion companies, and prior authorization for services
  • Help coordinate preventative services for defined populations, including education, vaccinations, monitoring, and arranging for lab tests and referrals
  • Document services in medical records, including all appropriate activities and plans
  • Obtain and deliver lab reports once reviewed by providers
  • Assist with sample management and necessary forms for alternative insemination
  • Collect lab samples, including use of phlebotomy, packaging, and providing for transport to labs
  • Attend to healthcare issues of specific populations

Maintaining Comprehensive Knowledge:
  • Stay up-to-date on medical issues, including acute and chronic care, especially HIV/AIDS and behavioral issues
  • Seek out continuing education opportunities related to essential job functions
  • Complete continuing education requirements per Massachusetts Board of Nursing regulations
  • Understand and maintain universal precautions in all clinical activities

Providing Health Education:
  • Provide community health education
  • Provide patient education related to medical and psychosocial needs
  • Maintain comprehensive knowledge of vaccines, medications, and other therapeutic interventions as appropriate to our outpatient clinic
  • Maintain comprehensive knowledge of Fenway Health policies, procedures, and services, including those of TFI
  • Maintain knowledge of community resources
  • Assist in training other employees in the medical department, including orientation of new staff, volunteers, and students

Requirements:
  • Current licensure as an LPN in the Commonwealth of Massachusetts
  • Current CPR certification
  • Ability to work harmoniously with diverse groups of individuals
  • Ability to work harmoniously and effectively with colleagues, patients, clients, and vendors across the spectrum of diversity

Preferred Qualifications:
  • IV insertion/phlebotomy skills
  • Supervisory experience
  • 1-year experience in an ambulatory care setting
  • Experience with patients with HIV and electronic medical records
  • Interest in LGBT health and commitment to community health
  • Experience working with an ethnically, culturally, and racially diverse environment

Working Conditions and Physical Effort:
  • Work involves considerable exposure to unusual elements, such as extreme temperatures, unpleasant odors, and/or loud noises
  • Moderate physical activity requires handling average-weight objects up to 15-20 pounds, assisting with patients, and standing and/or walking for most of the day
  • May work with blood or blood-borne pathogens and will require OSHA training
  • Work environment involves exposure to potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment
