Responsibilities:

  • Providing high-quality, patient-centered care to clients in their homes
  • Working collaboratively with our clinical team to develop and implement individualized care plans
  • Documenting client progress and communicating with families and healthcare providers as needed

Requirements:

  • Valid New Jersey LPN License or Multistate License
  • Physical from within three years
  • PPD or Chest X-Ray
  • Valid BLS CPR card (obtained in person not online)
  • Valid driver's license
  • G-tube, trach, vent experience or willingness to train
