Job Title: Certified Diabetes Care and Education Specialist

Brockton Hospital is seeking a highly skilled Certified Diabetes Care and Education Specialist to join our team. As a key member of our endocrinology department, you will play a vital role in providing diabetes education and support to patients with Type 1 and Type 2 Diabetes Mellitus, Pre-Diabetes, and Gestational Diabetes.

Responsibilities:
  • Provide diabetes self-management training and support to patients, teaching the diabetes 7 self-care behaviors and adhering to the ADA Standards of care.
  • Collaborate with endocrinology providers and function as a member of the care team to prevent or reduce acute and chronic complications of diabetes, reduce diabetes-related hospital admissions, and improve quality of life for those living with diabetes.
  • Maintain Diabetes Self-Management Education and Support (DSMES) Program Accreditation with the ADA, overseeing and implementing collaborative DSMES services, including evidence-based service design, delivery, evaluation, and continuous quality improvement, according to the National Standards for Diabetes Self-Management Education.
  • Develop and implement diabetes class curriculum and teaching materials, administering diabetes education classes, and reviewing program content annually.
  • Serve as a liaison between staff involved with the DSMES program, other Signature Healthcare departments, and administration.
Requirements:
  • Certification by the National Certification Board for Diabetes Care and Education is required.
  • Registered Dietitian or Registered Nurse licensed in the state of Massachusetts.
  • Computer skills to include Outlook, Word, Excel, Continuous glucometer and insulin pump software.
Work Environment:

This is a full-time 40 hour/week hybrid position, with three days in office and two days at home.

Location: Brockton Hospital, 110 Liberty Street, Brockton, MA.
