As a Hospice LPN, you will play a vital role in providing direct, skilled nursing services to patients in their homes, focusing on comfort, peace, and dignity. Key Responsibilities- Provide direct, skilled nursing services to patients in their homes, as outlined in the Plan of Care under the direction of the RN Case Manager.
- Build trusting, compassionate relationships with patients and their families, empowering them to make informed decisions about their care.
- Collaborate with the care team to provide comfort, peace, and dignity to patients, addressing their physical, emotional, and spiritual needs.
- Empower caregivers by educating them on care for their loved ones, promoting a supportive and inclusive environment.
- LVN (Licensed Vocational Nurse) or LPN (Licensed Practical Nurse) licensure.
